Licensed Practical Nurse : The LPN is responsible for providing high quality care to patients on an hourly basis to achieve palliation and/or manage acute medical symptoms through follow up visits and continuous care.
On-Call Registered Nurse : An On-Call Registered Nurse (RN) is responsible for responding to telephone requests for assistance, and performing home visits for hospice patients/families if required, after hours and on weekends. Utilizing physicians' orders and professional skills, the On-Call RN ensures they meet each patient's specific needs and is in compliance with CHAP, Federal and State regulations.
Social Worker (Part Time / Three Day Week) : LCSW and NYS License required. Social Work completes initial and ongoing assessment of patient and their families. These services are provided in accordance with the Interdisciplinary Team (IDT) plan of care.
