Quality Improvement Specialist- P/T : A Registered Nurse (RN) who supports the Quality Improvement Program through analysis of data, utilizing appropriate QI tools. Utilizes appropriate spreadsheets to display graphics in a meaningful way. Provides appropriate education and follow up. Covers for QI Coordinator in their absence.
Counseling Resource Clinician - P/T : Responsible for providing support to clinical SW/Spiritual Care management of cases. Responsible for evaluating department member's clinical performance on an annual basis. Masters Degree in social work. Licensed Clinical Social Worker. Knowledge of End-of-Life theory/practice. Five Years supervisory experience.
Certified Home Health Aide : Available to work Monday through Friday, 8:30am to 4:30 pm, to provide quality end of life care, including personal care and companionship to our patients. Must possess a Home Health Aide Certificate, Drivers license and have a vehicle in good working order. $20 per hr + benefits
